    Overview

    JHD Healthcare Partners partnered with a pediatric healthcare system seeking to reposition its physician enterprise for long-term success in an evolving reimbursement environment. Over time, the system had developed a fragmented physician structure through acquisitions and organic growth, resulting in multiple employed physician organizations operating independently, limited engagement with community physicians, and inconsistent care and performance expectations across the network.

    JHDHP was engaged to design and implement a future-state Target Operating Model that would align employed and community physicians, consolidate operations where appropriate, and create the capabilities required to succeed under value-based reimbursement.

    Approach

    JHDHP worked closely with system leadership to define a unified physician network strategy that clarified the roles of employed and affiliated physicians across the care continuum. This included strengthening engagement models for community physicians while consolidating employed physician organizations into a more integrated structure with streamlined governance, clearer accountability, and reduced administrative duplication.

    The Target Operating Model established standardized expectations for care delivery, operations, and performance across the physician enterprise. JHDHP also designed the operating capabilities required for value-based care, including population health management, care coordination, analytics, and performance management, ensuring the model could scale as reimbursement models evolved.

    Throughout the engagement, JHDHP supported change management and leadership alignment to ensure the model could be operationalized, not just designed, and embedded into day-to-day physician and administrative workflows.
